This website includes tools and advice for developing a patient engagement strategy from communities that participated in the Robert Wood Johnson Foundation’s Aligning Forces for Quality initiative (AF4Q). AF4Q aimed to build and support consumers’ capacity to manage their health and health conditions. AF4Q offers a wealth of lessons learned on patient experience, from developing a successful strategy to tailoring to partnering with community connectors.
